North Dakota’s Department of Health has notified First District Health Unit of NEW immunization requirements for 11th & 12th grade students enrolling in the 2018-2019 school year. To help ensure your child is eligible for enrollment, we will be holding a clinic at your child’s school.
Currently, MCV4 and Meningococcal B, are the two vaccines that can help protect your child from infection. Most children receive their initial dose of the MCV4 vaccine at age 11. It is now required that all enrolled students receive a second dose on or after his/her 16th birthday. Although a second dose of the MCV4 was always recommended, proof the vaccine was received is now a requirement for enrollment. Although Meningococcal B is not a requirement for enrollment, it is strongly recommended that your child receive both vaccines for better protection against this deadly disease.
Your child may be due for other mandatory vaccines. Students in grades 7th-12th are required to be up-to-date with the following vaccines: Tdap, Varicella (Chickenpox), MMR, Polio, and Hepatitis B. We will offer these vaccines the day of clinic. If you would like your child vaccinated for other mandatory vaccines, be sure to include these on the back of the consent form.
Requirements for school enrollment must be completed by October 1, 2018.
IMPORTANT! PLEASE CALL YOUR INSURANCE TO VERIFY COVERAGE FOR DAY OF SERVICE. EVEN IF YOUR CHILD HAS HAD COVERAGE IN THE PAST, YOUR POLICY MAY HAVE CHANGED. FIRST DISTRICT HEALTH UNIT MAY NOT BE IN YOUR NETWORK. ANY BALANCE THAT INSURANCE DOES NOT PAY WILL BE YOUR RESPONSIBILITY.
If you wish to have your child vaccinated at the school clinic, please return Vaccine Consent to your child’s school on or before the date of clinic. Please contact our office, 701-852-1376, for more information or help completing forms.
