Tanning Regulations

First District Health Unit will be providing license and inspection for Tanning Facilities in the seven county area.  The regulations were composed by the ND Legislature in 2007 (ND Century Code) and the Dept. of Health composed their rules, fees and license structure (ND Administrative Code).  Below please find a link for each of these codes as well as a copy of the license application, inspection form, an injury report form and a draft copy of the minors consent form.
